Clinical Pharmacist

• Review and manage patient medication therapy for safety, efficacy, and appropriateness • Participate in medical rounds with healthcare teams (physicians, nurses, specialists) • Provide medication therapy management (MTM) and patient counseling • Monitor patient outcomes and adjust drug regimens based on labs and clinical response • Identify, prevent, and resolve medication-related problems and adverse drug events • Provide drug information and evidence-based recommendations • Assist with antimicrobial stewardship, anticoagulation, pain management, or chronic disease management • Ensure compliance with hospital policies, safety protocols, and regulatory standards • Educate healthcare staff on medications, guidelines, and best practices • Document clinical interventions and outcomes in the electronic health record (EHR) Job Types: Full-time, Permanent Pay: ₹15,000.00 - ₹25,000.00 per month Work Location: In person
