Clinical Pharmacist

**JOB DESCRIPTION**: - Review and verify prescriptions to ensure accuracy, appropriateness, and safety. - Identify potential adverse reactions, and make necessary interventions. - Educate patients on the proper use of their medications, including dosage, side effects, interactions, and storage. - Address patient concerns and queries regarding medication adherence and potential side effects. - Work closely with physicians, nurses, and other healthcare providers to design effective medication practices. - Offer clinical advice and guidance on medication selection, dosing, and potential interactions based on patient-specific needs. - Ensure compliance with relevant pharmacy laws, regulations, and accreditation standards. - Maintain accurate patient records and documentation in accordance with hospital policies and legal requirements. - Participate in clinical audits and quality improvement initiatives to enhance patient safety and service delivery. - Stay updated on the latest evidence-based practices and contribute to continuous improvement in clinical pharmacy services. • *REQUIREMENTS**: - **Qualification**: Pharm D / M.
