Our work focuses on short-term social care reablement, using a strengths-based approach to promote independence, wellbeing, choice, and dignity. We support individuals during key transitions such as hospital discharge, crisis situations, or carer breakdown, helping them regain confidence and resilience in daily living. As a Reablement Assistant, you’ll deliver short-term reablement using a strengths-based approach to promote independence, wellbeing, choice, and dignity. You’ll work closely with individuals and their carers to support hospital discharge, prevent admissions, and help people regain confidence in daily living. You will work on a two-week rota pattern to cover the needs of the service which includes weekends, bank holidays and unsocial hours. Part time hours considered. In this role you will: - Support people in their own homes to achieve personal goals and regain independence. - Deliver reablement programmes prescribed by professionals. - Assist with personal care, mobility, nutrition, hydration and medication (training provided). - Provide advice and connect people to community resources. - Maintain accurate records and uphold safeguarding principles. - Work collaboratively with health and social care partners. About you: Essential criteria for this role: - Experience or ability to work with vulnerable adults in an Adult Social Care setting. - Knowledge of care in the community and CQC standards. - Ability to follow reablement plans and promote independence. - Strong communication and problem-solving skills. - Resilience and ability to work independently in the community. - Willingness to work towards Level 2 Adult Social Care Certificate if not already held. - Functional skills in English and Maths (Level 1). - Ability to meet the travel requirement of the role. What we can offer you: Salary: The salary band for this role is £28,792 - £31,022 pro rata, per annum. The starting salary is £28,792 with annual progression through the salary bands.
