Clinical Pharmacist (Mira Road)

- To work with the hospital information system and other information technology resources and software as may be assigned on a periodic basis from time to time as per attached appendix - Prescription Audit for dose, dosage regimen for medication errors (e.g.: Nursing administration to patients, Pharmacy dispensing to wards) - Discharge patient audit (Dose and drugs prescribed to discharged patients) - Patient counselling about drug, dose and disease to patients - Dose correction to patient according to disease status and patient weight. - Drug interactions and allergies - Drug information to Nurses, RMO’s and Pharmacy - Adverse drug reaction finding and documenting - Drug – drug interaction - Checking antibiotic regimen - Counsel discharged patients about anti coagulants - Current drug information to be shared with PTC - Presenting new drug information to PTC committee - Medication therapy management - Ambulatory care - Drug information service - Awareness about common drugs to hospital about safety - Ward round participation - Auditing storage of Narcotics/ High alert medication/ LASA medicines - Preparing and updating hospital formulary - Training Maintaining quality indicators Job Types: Full-time, Permanent Pay: ₹20,183.68 - ₹26,125.09 per month Work Location: In person
