Clinical Pharmacist

• JOB DESCRIPTION**:
• Review and verify prescriptions to ensure accuracy, appropriateness, and safety.
• Identify potential adverse reactions, and make necessary interventions.
• Educate patients on the proper use of their medications, including dosage, side effects, interactions, and storage.
• Address patient concerns and queries regarding medication adherence and potential side effects.
• Work closely with physicians, nurses, and other healthcare providers to design effective medication practices.
• Offer clinical advice and guidance on medication selection, dosing, and potential interactions based on patient-specific needs.
• Ensure compliance with relevant pharmacy laws, regulations, and accreditation standards.
• Maintain accurate patient records and documentation in accordance with hospital policies and legal requirements.
• Participate in clinical audits and quality improvement initiatives to enhance patient safety and service delivery.
• Stay updated on the latest evidence-based practices and contribute to continuous improvement in clinical pharmacy services.

• Qualification**: Pharm D / M. Pharm
• Registration
• Experience**: Above 2 Years

Pay: ₹18,000.00 - ₹20,000.00 per month
