To work with the hospital information system and other information technology resources and software as may be assigned on a periodic basis from time to time as per attached appendix
Prescription Audit for dose, dosage regimen for medication errors (e.g.: Nursing administration to patients, Pharmacy dispensing to wards)
Discharge patient audit (Dose and drugs prescribed to discharged patients)
Patient counselling about drug, dose and disease to patients
Dose correction to patient according to disease status and patient weight.
Drug interactions and allergies
Drug information to Nurses, RMO’s and Pharmacy
Adverse drug reaction finding and documenting
Drug – drug interaction
Checking antibiotic regimen
Counsel discharged patients about anti coagulants
Current drug information to be shared with PTC
Presenting new drug information to PTC committee
Medication therapy management
Ambulatory care
Drug information service
Awareness about common drugs to hospital about safety
Ward round participation
Auditing storage of Narcotics/ High alert medication/ LASA medicines
Preparing and updating hospital formulary
Training

Maintaining quality indicators
