Job Profile Summary
Manages a portfolio of clinical programs delivered by both internal Millennium teams and external clinical partners, each designed to connect defined patient populations with specialized services. Owns program operations from implementation through ongoing performance management, including workflow design, KPI development, provider education, patient triage processes, and issue resolution.
Works across clinical, operational, analytics, quality, technology, and partnership teams to ensure each program is effectively integrated into provider and patient workflows. Monitors results, identifies performance gaps, and drives corrective action. For externally operated programs, serves as the designated relationship owner to coordinate with the partner on performance, action plans, and governance.
Job Description
KEY RESPONSIBILITIES
- Lead the launch, implementation, expansion, and optimization of clinical programs across markets and provider teams.
- Translate program objectives into clear workflows, roles, milestones, service expectations, and escalation processes.
- Manage the day-to-day operations and performance of assigned internal and partner-enabled clinical programs.
- In collaboration with Medical Economics, establish KPIs and reporting across enrollment, engagement, provider adoption, service delivery, clinical outcomes, quality, and patient experience.
- Monitor program performance, identify root causes of gaps, and lead corrective actions and improvement efforts.
- Serve as the operational point of contact for external partners, coordinating workflows, reporting, issue resolution, and follow-through, including supporting Joint Operating Committee and other governance activities and associated performance materials, action tracking, and risk escalation.
- Assist with the operational triage and routing of patient cases, escalating clinical questions to the appropriate clinician or care team.
- Educate MPG providers and care teams on program eligibility, available services, referral processes, and clinical workflows.
- Develop and maintain standard operating procedures, referral guides, provider education materials, and other program documentation.
- Gather feedback from providers, patients, partners, and frontline teams to improve program effectiveness and usability.
- Coordinate cross-functional dependencies and communicate program performance, risks, and recommendations to stakeholders.
QUALIFICATIONS
- Bachelor’s degree required; degree in healthcare administration, public health, nursing, business, operations, or a related field preferred.
- Four or more years of experience in management consulting, healthcare operations, population health and care management, value-based primary or specialty care, or related work.
- Demonstrated ability to establish KPIs, analyze performance, and use data to drive operational improvements.
- Strong project and program management skills, with the ability to independently manage multiple priorities
- Strong written and verbal communication skills, including the ability to translate clinical, operational, and analytical information for different audiences.
- Ability to lead through influence, coordinate cross-functional stakeholders, and drive accountability in a matrixed organization.
- Experience in a medical group, accountable care organization, health plan, specialty-care organization, healthcare technology company, or healthcare consulting environment is preferred. Particularly relevant experience includes managing clinician-facing programs, designing workflows and operating processes, supporting external partnerships and governance, or working with complex chronic conditions, serious illness, clinical pathways, referrals, care coordination, or utilization management.
- Clinical licensure, MPH, MHA, MBA, PMP, or a related credential is a plus.
- Florida location or Florida healthcare-market experience preferred; remote candidates will be considered.
